“Ozempic Face” is a term that is showing up more in public spheres, describing facial changes that can happen with rapid weight loss from GLP-1 medications like Ozempic and Wegovy. Ozempic face is a term used for the aged appearance that can develop after rapid weight loss on GLP-1 medications. Changes are often seen in the cheeks and temples, or under the eyes, where the skin is more fragile. It is not an indication of overall health, but it can make a person look more gaunt and tired. The term can apply to facial changes due to the use of any weight loss medication, not just Ozempic.

The face contains fat pads that hold up the cheeks and support the areas around the eyes. These fat pads keep the face looking full and youthful. GLP-1 medications cause fat loss throughout the body, and the pads in the face shrink along with other areas. 

Skin Elasticity and Collagen

Skin is flexible and can stretch with weight gain and contract when weight is lost. However, it can only do this to a point. Once collagen and elastin production slows, the skin loses some of its ability to bounce back. Beyond the natural skin changes that occur with age, rapid weight loss may actually be the cause of reduced skin elasticity. One study by ePlasty suggests this connection, as those who lost weight quickly showed skin that was weaker than that of patients with morbid obesity. 

The Role of Weight Loss Speed

Gradual weight loss gives the skin months or years to retract slowly. GLP-1 medications can cause patients to drop 15 to 20 percent of their body weight in under a year. While there can be benefits for overall health in dropping weight quickly, this is far faster than the skin can keep up with. The faster the loss, the more loose tissue is likely to show up in the face.  

Common Signs of Ozempic Face

  • Hollow Cheeks: The midface often loses its fat pads first. The cheeks can begin to look flat or sunken as the underlying bone structure becomes more visible. 
  • Under-Eye Hollowing and Dark Circles: Fat loss around the eye socket creates a sunken tear trough that casts a shadow. This can cause dark circles and a tired look.
  • Jowling and Jawline Sagging: As midface fat shrinks, skin descends toward the jawline, causing jowls that form at the corners of the mouth. 
  • Loose Skin Along the Neck and Lower Face: Skin on the lower face and neck that has been stretched by extra weight may not snap back on its own after the fat is gone. This can cause a turkey neck or folds on the neck. 
  • Deepened Nasolabial Folds and Marionette Lines: The lines that surround the mouth often become deeper as they lose the support of underlying fat pads.
  • Droopy Eyelids and Hooded Eyes After Weight Loss: Thinning fat above and around the eye, paired with skin that no longer holds its shape, pulls the brow down and lets the upper lid fold over itself. The eyes may look smaller or more tired, and the lash line may disappear under a layer of loose skin. 

How Ozempic Face Differs From Normal Aging

Normal aging happens slowly. Collagen breaks down a small amount each year and fat pads thin gradually, so the face has time to adjust. Ozempic face compresses years of volume change into months. The result is a face that can look a decade older within a short window, even when overall health has improved. Weight loss patients often seek cosmetic care earlier than they would have otherwise, because the visible changes outpace their actual age.

Who Is Most Likely to Notice These Changes

  • Patients Over 40: Skin elasticity starts to drop in the late 30s and falls more steeply after 40. Patients in this age range have less reserve to handle rapid volume changes, so the loss is more noticeable.
  • Patients With Higher Starting BMI: The more weight a patient loses, the more facial fat goes with it. Patients who start at a higher BMI tend to see bigger changes in the cheeks, temples, jawline and neck once the weight is off.

Will Ozempic face get worse if I keep taking the medication?

Facial changes tend to plateau once your weight stabilizes. If you stay at a steady weight, you should not see continued loss in the face even with ongoing GLP-1 use. The medication itself does not specifically target facial fat. Patients on a maintenance dose with stable weight often see their results from cosmetic treatments hold well over time.

Can I treat Ozempic face before I finish losing weight?

We usually wait until weight has stabilized before performing surgery, since further loss can change the result and may leave the face looking off-balance. Some skin tightening and lighter treatments can be done earlier and may even help the skin adapt as the weight comes off. Is Ozempic face permanent?

Volume loss and skin laxity do not reverse on their own. Without treatment, the changes tend to stay or progress as the patient ages, since natural aging adds further collagen loss on top of what GLP-1 weight loss has already caused. Fillers, laser treatments and surgery can correct the appearance, and many patients are surprised by how much can be restored once a plan is in place.

Does everyone who takes GLP-1 medications develop Ozempic face?

Younger patients with strong skin elasticity and slower weight loss often see only minor changes. Risk goes up with age, total weight lost and how quickly the loss happens. I have had Botox, fillers, and IPL laser. I’m 38 and was noticing that I often look tired, with under eye bags, a bit of smile lines, and slightly droopy eye brows. I noticed right away that the Botox did great with lifting up my brows. The fillers gave me some cheeks and disguised the under eye bags.. my laugh lines are much less noticeable. I still have 2 more sessions of the IPL laser but so far I’ve noticed much less redness and discoloration and look forward to the final result. I did feel like Dr. Zormier cared about making sure I was comfortable. I noticed that some of the reviews mentioned that she wasn’t sensitive to pain- that was not my experience at all. If I had any criticisms- I feel like there might be a bit more fillers than I would have liked in my cheeks.. this was my first experience getting work done so perhaps I didn’t communicate with DR. zormier as thoroughly as I should have. I didn’t know what to communicate so I pretty much let her do what she thought was best. Next time I have fillers, I will ask for about 25% less than what was injected. Also, for the fillers , Botox , and IPL laser, I spent $6000 . Very expensive . I’m hopeful that maintaining is much less than this .
